FLSA Rights / Consent to Sue

Workers’ Rights Under the Fair Labor Standards Act (FLSA)

The Fair Labor Standards Act (FLSA) requires qualified current and former employees who wish to participate in the Supercuts lawsuit to “opt in” or request to join the case within a specific time frame.

If you worked or currently work for a Supercuts franchise in Arizona as a Stylist, Key Holder or Shift Manager and are determined to be eligible to join the lawsuit, you must return a completed “Consent to Sue” form as soon as possible in order to be included in the claims, under the FLSA, seeking unpaid overtime.

If you have worked for Supercuts, Terrace Management, HF Consulting, PHX SC MANAGEMENT, INC, or the HF SAUNDERS CO, LLC as a Stylist, Key Holder or Shift Manager and were not paid for all of the hours you worked, you may be able to join the case.

Consent to Sue

If you think you may be eligible to participate in the lawsuit and want to do so, please complete the Consent to Sue form and send it to us to review. Upon receipt of the form, an attorney will contact you and confirm that your form has been received and reviewed and that you may be eligible to join the case.

The Consent to Sue is not valid until you have received a letter from Martin & Bonnett, PLLC verifying that your Consent to Sue has been filed with the Court. If you have not received a letter within three weeks of submitting your form, please call Martin & Bonnett at 602-240-6900 or 800-952-4750.

Please print this form and make sure that your handwriting is legible so we are able to contact you as needed.
